I like that this incorporates all factors that could influence the student's behavior in the “classroom considerations” section. The way a classroom is arranged can definitely affect students' behavior. It's beneficial to include the “instructional format” because the student could be acting out due to difficulty learning in a specific way. For example, if the student feels uncomfortable doing things in front of the class; he or she could be acting out during “demonstration” activities.

I love the idea of students self evaluating their own behavior. I think it's beneficial for students to have a chance to reflect on their classroom behavior and it is helpful for the teacher to see how the student's evaluation of his/her behavior compares to their own.

Response to Intervention: The Ultimate Social Skills Checklist « The Helpful Counselor  I found this chart to be especially helpful in tracking behavioral issues of my younger students (1st and 2nd grade). I like how specific it is in that it provides different activities dependent on play level (beginner/int/advanced). I think this is particularly useful in that it can track an individual students' progress throughout the year as they are growing and engaging in more advanced activities.
